Experience
IRW, one of the largest post-acute rehab providers in the region, currently employs more than 120 physical, occupational and speech therapists, providing services in skilled nursing facilities (SNFs), home health, assisted living, independent living and outpatient settings in the Omaha and Lincoln metro areas. Approximately one-third of these therapists have advanced credentials in their field – many with doctorate level degrees – and IRW employs more therapists board-certified in geriatrics than any organization in the region. In recent years, team members have been published in peer-reviewed rehab journals and have presented our work at national conferences.
With an actual focus on innovation, IRW collaborates with Creighton University School of Pharmacy and Health Professions for both the Occupational Therapy Fellowship Program in Gerontology and the Physical Therapy Geriatric Residency. The Physical Therapy Residency Program is one of only 16 programs in the country and the only program in Nebraska or Iowa, while The Occupational Therapy Fellowship Program is the very first program of its kind in the country.
Leadership
Each leader of Innovate Rehab & Wellness (IRW) has decades of experience in physical therapy and rehabilitation services. Together they lead a team of more than 100 physical, occupational and speech therapists. Their strategic vision for serving aging adults has led to the creation of several unique programs and professional relationships that enhance the services that IRW offers. Here’s more about our leadership team:

Matt Oestmann, DPT, MBA – Vice President of Rehab & Post-acute Services
Matt Oestmann is Vice President of Rehab and Post-Acute Services for Innovate Rehab & Wellness. His accomplishments have included building a strong team of dedicated therapists with decades of experience in home health and orthopedics. He is active in American Physical Therapy Association and National Association of Rehab Agencies, serving on various committees over the years. He has also served the Nebraska Home Care Association as a Board Member and Legislative committee member. He was selected as one of the Midlands Business Journal’s “40 Under 40” in 2016. Dr. Oestmann received his Bachelor’s degree in Biology and Psychology from Dana College and his Masters in Physical Therapy and his Doctorate in Physical Therapy from UNMC. He earned his MBA at the University of Nebraska at Omaha in 2019. Dr. Oestmann is a Nebraska native and currently lives in the Papillion area with his wife, Mylissa, and their five children.

Grace Knott, PT – Administrator of Post-acute Rehab Services
Grace Knott is the Administrator of Post-acute Rehab Services for IRW. She is the president of the Nebraska Physical Therapy Association and has extensive experience in acute, skilled nursing and outpatient settings as both a clinician and a manager. A graduate of Kansas University in physical therapy, her passion is working with the older adult, especially in the areas of fall risk reduction, dementia care and optimal aging.

Raegan Muller, PT – Regional Director of Rehab
Raegan Muller is the Regional Director of Rehab for Innovate Rehab & Wellness. She attended Concord University in Seward, Nebraska, where she played basketball and softball while obtaining her Bachelor of Arts degree. She graduated from UNMC with her Masters in Physical Therapy in 2001 and her board certification in Geriatrics from the American Physical Therapy Association (APTA) in 2012. She is the Residency and Fellowship Special Interest Group chairperson for APTA Geriatrics and the co-chairperson for the APTA-Nebraska Continuing Education Committee. She is also a member of the UNMC clinical advisory panel for physical therapy education. She lives in Bellevue with her husband and two teenage children. She enjoys a variety of sports, fresh air and running marathons.
History
Innovate Rehab & Wellness (IRW), an affiliate of Hillcrest Health Services, has been providing therapy to the aging adult since 1997. Locally owned and operated, IRW is led by experienced therapists who have dedicated their careers to providing rehabilitation services to aging adults. We have invested in attracting and training the most skilled geriatric therapists in the Midwest through continuing education, innovative rehab programming, state of the art technology, and partnerships in the rehab community.
